Youth gaming sessions — ages 13–17

Parent or Guardian Approval

ReRokc offers scheduled online gaming sessions for teens ages 13–17. A parent or legal guardian must approve participation before a youth booking is confirmed. Children under 13 are not eligible.

1. Please do not enter your child's home address, school, account password, or other unnecessary personal information.

2. Required parent or guardian approval I confirm that I am the participant's parent or legal guardian, have authority to approve their participation, and am at least 18 years old. I confirm that the participant is between 13 and 17 years old. I approve participation in the game and session listed above. I have reviewed the game's age rating, content, communication features, and applicable account requirements. I understand that online games may include interaction with other players and exposure to language or content that ReRokc cannot fully control. I will review the participant's privacy and parental-control settings and remain available during the session. I understand that youth sessions do not include betting, wagers, or gambling. Participation does not require signing up for a gambling service. I agree to handle booking communications and any session payments. Any fee and cancellation/refund terms must be shown to me separately before payment. This approval does not authorize recurring charges or additional purchases. I have reviewed the ReRokc Community Rules and Privacy Notice, including how to report a concern and request access to, correction of, or deletion of participant information.

3. Participant agreement (completed by the teen) I want to participate. 4. Optional recording and public sharing

Optional: saying no will not prevent participation. Leave all choices unchecked unless permission is intentionally granted.

I permit ReRokc to record this session's gameplay, including the participant's gamer tag and any voice captured through the session. I separately permit ReRokc to edit and publicly share the approved recording or clips on the specific channels listed below. Approved channels Participant: I also agree to the recording choices selected above.

This permission applies only to the session listed on this form. It does not authorize use of the participant's real name, face/webcam image, school, location, or private contact details, and does not authorize paid advertisements, sponsored endorsements, or monetization. Publicly shared material may be copied by others; ReRokc cannot guarantee removal of copies outside its control. If either the guardian or participant declines, ReRokc will not intentionally record, livestream, or publish the participant through its own session-production tools.

5. Withdrawing approval or reporting a concern

Contact ReRokc at Chris@rerokc.com to cancel participation, withdraw future recording permission, report a safety concern, or request removal of content under ReRokc's control. Withdrawing recording permission stops future authorized use.

6. Signature

By signing, I confirm that the information above is accurate and that I approve the required participation terms. Optional permissions apply only where selected.
